Chief Canfield has announced the Unique Needs Awareness Program, aimed at seeking greater understanding of adults and children more vulnerable to wandering, going missing and having trouble communicating with first responders. Forms are now available for parents and/or legal guardians to fill out with information about a person who may have chronic mental illness, be unable to communicate, have a developmental disability or suffer from dementia or some other form of memory loss.
An online fillable form allows the information to be send directly to the police department where it will be store in a database of information. This form allows for the individual's photograph to be easily uploaded. We strongly suggest that you create a free account login for the City's website prior to completing the form, as this will allow you to save the form's information and return to the form for completion if the need arises.
